Independence subscribers will be receiving tax forms to assist them with their 2019 tax year filings. Three separate forms will be either mailed or available online by January 31.
Form 1095‑B for proof of minimum essential coverage
This form provides verification to subscribers that they had minimum essential health care coverage in 2019 as required by the Affordable Care Act. This form is mailed to subscribers only, unless we receive a request from a subscriber to issue a duplicate form to an enrolled spouse and/or dependent. Additional information on the 1095‑B form is available at ibx.com/1095.
Subscribers can also download a copy of their 1095‑B through their member account on ibx.com. We will be reaching out to registered subscribers via IBX WireTM text messaging and email regarding the availability of the 1095‑B forms at ibx.com.
