As a reminder, it is that time of year for you to distribute information to anyone on your group who is eligible for Medicare that your prescription plan is “creditable.” Attached is the model notice.
The only information that you must fill in: the space for (entity) is the insurance company, e.g. Keystone, Aetna; the name of the plan would be your employer name; further down you must include a name/phone number of a person at your organization who they can contact for more information.
There is an ‘optional insert’ at the bottom that you DO NOT have to complete (it gives you the option of personalizing the notice). The attached notice should be distributed to those on your group eligible for Medicare by October 15th.
